The sample must be correctly identified: identification sample/patient and date of extraction.
The sample must be accompanied by the correctly completed application de Análisis:
-
data of the applicant: identification of the doctor/person authorised to apply for the analysis, centre/hospital, addressee and contact for sending the report results.

data of the patient: patient identification, sex and date of birth.
-
data from sample: identification of sample, subject from sample primary (if applicable anatomical site of origin) and date of removal.
-
data relevant clinical findings: diagnostic suspicion, time of diagnosis, relevant individual and/or family information, etc.

Analysis requested in the petition.

Informed consent (IC). The ICs must be obtained by the physicians of the centres where the samples are taken. It is possible to keep and/or use the samples for research purposes, for which an additional IC is sent together with the application Analysis (template PT-CLD-001/PL02).
Samples should be sent:

In a suitable container to protect its integrity (Packing Instruction P650, transport designation UN 3373).

It is recommended to label the consignment as "refrigerate at destination".

Under the conditions established for the subject of testanalytical requested (see table).

We recommend shipment at room temperature (with some exceptions), within 24 hours of collection. If shipment is delayed by +24 hours, we recommend keeping the samples at 4°C until shipment (except for paraffin-embedded or fixed tissue, which is kept at room temperature).
